How to Get Your Records from Lower Manhattan DMV

You have two options for obtaining your records.

In Person

You can go to the Lower Manhattan DMV office with the following documentation to obtain records:

  • A completed Request for Driving Record Information
  • Your driver’s license, government-issued identification, or six different points of identification (You can call the DMV to determine what counts toward your identification if you don’t have a license or government-issued ID.)
  • The $10 search fee, payable via cash, credit card, check, or money order (checks and money orders made payable to “Commissioner of Motor Vehicles”)

Online

You can request your driving record online:

    • Through your NY.gov ID account: You can obtain a PDF of your driving record for $7 by signing into your NY.gov ID account. If you need to create an account, you will need your client number and document number on your ID. (The NYS DMV provides sample documents to help you find these.) You also need your date of birth, the zip code and state of the address the DMV has on file for you, and the last four digits of your Social Security Number.
  • Without an account: You can also order your driving record without an account. You’ll need the same information as above.
  • With the Records Request Navigator: If you don’t have an account or an ID, you may use the Records Request Navigator. It can take five business days for a request to process.

How Do I Get a Crash Report?

You also have two options to get a report after an accident in Lower Manhattan:

  • Online: You can order your crash report by using your NY.gov ID account. To obtain the report, you will need to pay a $7 search fee and a $15 report fee. You can also use the Records Request Navigator and pay a $10 search fee and a $15 report fee.
  • By mail: You can get your crash report by mailing a Request for Copy of Crash Report (PDF) and payment to:
